Event Planning Worksheet (for events involving alcohol)
The purpose of this form is both to register on-campus events involving alcohol and to assist organizations in planning for safety issues during events. For events occuring in residential areas, students/organizations must contact the Area Coordinator responsible for the area in which the event will occur. For Events occurring on campus in non-residential areas, this form must be filled with the Student Activites office at least two weeks before the event. Approval of all events involving alcohol must be accompanied by a planning meeting with a representative of the Student Activities Office of the Office of Residence Life, thus sponsors who are seeking event approval for the first time must submit a completed registration form seven days in advance. After this, a 48 hour advance approval is required for events.

Note: All outdoor events must be approved by the Director of Student Activities.

In submitting this form:
1. We have read the Alcohol Policy and gree that our event will abide by all State laws, College requirements, and event registration guidelines.
2. We recognize that the College will not accept alcohol by use of our guests or us as an excuse for misconduct.
3. We agree that the event will follow the specifications of the approved Event Registration Form.
4. We agree that some of our sponsors are 21 years of age or older, and at least one sponsor, age 21 or older, will be present at all times. We understand that all sponsors, whether present or not, may be held accountable for any problems that occur.
5. We agree that only persons 21 years of age or older will purchase, consume, or be served alcohol.
6. We agree that bartenders will remain sober, will serve guests one drink at a time for their own personal consumption only, an will not serve anyone who appears intoxicated.
7. We agree that service of alcohol will stop one hour before the event ends.
