Enrollment Forms due by November 14, 2008

It is time once again for open enrollment in the Section 125 Flexible Spending Accounts (FSA) for medical or dependent care expenses. FSA accounts are exempt from federal income taxes, Social Security (FICA) taxes and, in most cases, state income taxes. You elect to have a certain amount of pay withheld on a pre-tax basis to cover out-of-pocket medical expenses. Examples of eligible expenses include co-pays, deductibles, eye glasses, laser eye surgery, contact lenses, braces/orthodontic services, and over-the-counter drugs that treat medical conditions. The FSA is also available for dependent care expenses, such as day care, day camp, eldercare, and before and after school care.
In order to enroll in the 2009 Section 125 Flexible Spending Account, you will need to return the Flexible Spending Account Enrollment Form to the Office of Human Resources by November 14, 2008. You can access the enrollment forms at https://www.flexdirect.adp.com/help/FSAinfo/learning/index.html
Just click on “Tools & Forms” (green tab, upper left). Both the health care and dependent care forms are in the Blue section titled "FSA Tools and Forms." The forms can be edited on line - just complete the enrollment form, print it out, then sign and return it to the Office of Human Resources, Box 4 by November 14th. Current participants must also complete and submit a new enrollment form in order to participate in 2009.
